Clear Lake Optometry accepts many health insurances. The list below is only a partial list, and it is highly likely that your insurance IS accepted by us.

Clear Lake Optometry also accepts CareCredit and Vision Care Direct as alternatives to insurance.
If you have an appointment with Clear Lake Optometry, review the following checklist:
- 1. Determine whether or not your individual insurance policy covers eye examinations, and whether or not a referral or authorization for the examination is required.
- 2. If you have an insurance authorization for an eye examination, make sure that the appointment is scheduled within the required time frame
(usually 30 to 90 days).
- 3. If you are a new patient, feel free to fill out and print the Patient Information Form to be filled out ahead of time. You do not need to hand write any of your information. Just place your cursor in the first box and use your tab key to navigate to the next box. When you have completed the form, print it out and bring it in to your next appointment.
To download and printout the appropriate form, you will need to have the Adobe Acrobat viewer installed in your browser. This plug-in is easily installed, and available for free from Adobe.
